We are committed to helping our contract holders live safely and independently in their homes and communities. We have our own dedicated adaptations team, and we will fund and maintain all adaptations. We cannot give equipment or make adaptations for short-term disabilities which are expected to improve, but we can offer advice and information.
We will always consider simple solutions first; like offering bathing equipment before considering major adaptations such as installing a level access shower.
How can we help?
Our team can recommend equipment and adaptations, which may help you to access your home and carry out day-to-day activities.
Who should you get in touch with?
Some minor works do not need an occupational therapy (OT) assessment and can be requested via our Customer Hub team. When you make your request, a referral to Care and Repair will be made for you. They will then contact you to arrange access to your home.
Examples of minor works include handrails, lever taps, and grab rails.
For all other requests please contact Call Torfaen on 01495 762200. They may give you advice, prescribe equipment, or refer you for extra minor adaptations at this stage. If you need an assessment for other needs, you will be put on a waiting list to be assessed by an occupational therapist. The timescale for this will depend on how urgent your need is.
If you have already made a referral and feel your case needs to be seen more quickly because your situation has changed since your original referral, you should call the Disabled People’s Service at Torfaen Council on 01495 762200
What happens next?
Once we have received your referral from Torfaen Council, we will contact you to arrange the best time to assess you and your home in line with our waiting list times. With your agreement, we may recommend the following:
• Equipment (for example, to help with getting in and out of the bath, on and off a chair, in and out of bed, etc.).
• Minor adaptations (such as handrails, grab rails, shower seat, etc.).
• Major Adaptations (such as ramps, stairlift, level access shower, etc.).
• Advice and support to move to a home that better suits your needs.
• Referral to other services.
All equipment is requested from Torfaen Council and is given as a long-term loan. Please let us know when equipment is no longer needed so it can be recycled. Adaptations cannot be given for the storage and upkeep of privately purchased wheelchairs, scooters, or walking aids. We can only offer these adaptations for equipment provided by the National Health Service.
What if my home isn’t suitable for adaptations?
If it is not possible to carry out the work then your occupational therapist will talk to you about other options. This could include helping you to move to a more suitable home.
